Who can take part
- Diagnosed with stage III locally advanced rectal cancer (adenocarcinoma type) confirmed by biopsy
- Cancer located in the lower rectum (less than 15 cm from the anus) and cannot be safely removed by surgery alone
- No cancer spread to other organs; willing and able to receive chemotherapy (5-FU based) and radiation
- Good overall health (ECOG score 0–1), with normal liver, kidney, and blood cell counts
- Not pregnant or breastfeeding; willing to use effective contraception if of childbearing age
- No prior pelvic radiation, rectal surgery, or certain genetic conditions (MSI-H/dMMR tumors)
