Who can take part

  • You are 18 years or older
  • You came to the emergency department with signs or symptoms of heart failure (like shortness of breath or swelling)
  • Blood tests show elevated natriuretic peptides—proteins that indicate heart stress (NT-proBNP >125 or BNP >50)
  • You have never been diagnosed with heart failure before
  • You are not currently receiving dialysis or kidney replacement therapy
  • You are not receiving comfort or end-of-life care
