Who can take part
- You identify as Black
- You are 18 years old or older
- You have a history of high blood pressure
- Your kidney function (measured by eGFR) is between 15–44, meaning moderate kidney disease
- Your blood pressure is elevated (systolic ≥120 or diastolic ≥80) but not dangerously high (below 180/110)
- You are stable on your current blood pressure medications with no changes in the past 4 weeks
