  • You are 18 years or older
  • You identify as Native Hawaiian or Pacific Islander
  • You have been diagnosed with type 2 diabetes
  • Your most recent A1C (average blood sugar) is 8% or higher
  • You have at least one social need (like help with food, housing, or transportation)
  • You receive your care at Kaiser Permanente Hawaii
  • You have an adult family member who agrees to attend the classes with you
