Triple therapy vs sildenafil dose increase for PAH

Pulmonary Arterial Hypertension (PAH)

This study compares two approaches for people with PAH whose current treatment (a combination of sildenafil and an endothelin receptor antagonist) needs to be stepped up. Half the participants will add a third medicine (iloprost or selexipag), while the other half will increase their sildenafil dose. The goal is to see which strategy works better for improving symptoms and lowering risk.

Who can take part

  • You are 18 years or older
  • You have been diagnosed with pulmonary arterial hypertension (PAH) by a doctor
  • You are currently taking a stable combination of an endothelin receptor antagonist and sildenafil
  • Your PAH risk is not low (intermediate-low, intermediate-high, or high risk)
  • Your doctor recommends increasing your treatment (therapeutic escalation)
  • You are not allergic to or have a condition that prevents using prostacyclin pathway drugs (like iloprost or selexipag) or increasing your sildenafil dose
