- You are living with heart failure and are being seen at a non-communicable disease clinic in one of these three hospitals: Butaro, Kirehe, or Rwinkwavu.
- You are able to understand and agree to take part in the study.
- If you are under 18, you must be at least 10 years old, and your parent or caregiver must also agree.
