- You are 18 or older
- You were recently in the hospital for heart failure
- You and your caregiver can read and speak Greek
- You have had heart failure for at least 6 months
- Your caregiver has been helping you for at least 6 months
- You are not currently dealing with serious mental health, alcohol/drug issues, dementia, or Alzheimer's
