  • You are 18 or older.
  • You have a body mass index (BMI) of 30 or more, or a BMI of 27 or more with a weight-related health problem.
  • You are a Mayo Clinic employee or a dependent of one.
  • You have been referred to the Community Internal Medicine Weight Management Program.
  • You are not pregnant or breastfeeding.
