- You must be 22 years or older.
- You already have a CardioMEMS heart monitor implant (or are scheduled to get one).
- Your CardioMEMS signal must be working well (70% or more signal strength) at the start.
- You must be able to use the Vivio device to take measurements at home.
- You must be willing to let the study team see your medical records for up to 2 years after the study ends.
- You must be able to understand and sign a consent form.
