Who can take part

  • You are 18 or older and receive regular outpatient care at the University of Chicago Medical Center
  • You are expected to live at least 3 more years
  • You take 1 to 6 prescription medicines now, or you are 65+, or you expect to need prescription medicines within 5 years
  • You may be especially likely to be invited if you have conditions like long-term blood thinner needs, inflammatory bowel disease, autoimmune/inflammation conditions, hepatitis C, or non-metastatic cancer, or specialized heart care
  • You should not have a serious condition expected to cause death within 3 years
  • You should not be getting ready for a liver or kidney transplant
  • You must be able to understand and give consent
