This study tests whether bariatric surgery (either sleeve gastrectomy or Roux-en-Y gastric bypass) improves severe nonalcoholic steatohepatitis (NASH) with advanced liver scarring in people who are obese. You may also need liver biopsies to confirm your liver disease before and after treatment.

Who can take part
- You can sign informed consent and follow the study plan and visit schedule
- You have NASH with advanced liver scarring (F3 or F4) confirmed by FibroScan and/or a liver biopsy
- You agree to have a liver biopsy during screening and another liver biopsy at the end of treatment
- Your body weight matches the study: BMI must be at least 30, with specific ranges based on F3 vs F4
- If you have cirrhosis, blood tests and liver function scores must be within limits (and certain complications must be absent)
- You must be able to safely have only sleeve gastrectomy or Roux-en-Y gastric bypass
