De-intensified radiation after surgery for HPV throat cancer

Who can take part

  • You have confirmed squamous cell cancer in the tonsil or base of tongue, or an unknown primary, plus an HPV signal in the neck lymph node (p16-positive)
  • Your tumor is HPV-associated (for example, p16 is strongly positive, or HPV tests like in-situ hybridization or PCR are positive)
  • You are planning curative-intent surgery, and the surgeon expects clear margins (no cancer at the edge of tissue)
  • Your cancer has not spread to distant sites (M0) and the tumor/nodes are early (T0–T2 and N0–N1)
  • You meet general health/lab requirements (including blood counts and normal-ish liver/kidney tests) and can do normal daily activities (ECOG 0–1)
  • You are 18+ and can sign consent; you also must not have had head and neck cancer in the last 5 years or prior head and neck radiation
