Blood pressure–guided care for older or frail surgical patients
Treatments studied
Part of Brain & nervous system, Injuries & trauma clinical trials.
This study tests a goal-based blood pressure treatment approach during major surgery for people at higher risk (often older or frail with high blood pressure). The goal is to see if this BRAIN-targeted strategy can improve how patients do after surgery.
Summary written for real people, not researchers, by Clin2.
Who can take part
- You have high blood pressure and are considered higher risk (age 65+ or frail).
- Your anesthesiologist plans to place an arterial line (a catheter in an artery) during anesthesia.
- You are scheduled for major high-risk surgery (such as abdominal, vascular, urologic, or thoracic surgery, including one-lung breathing) that lasts over 60 minutes.
- You can understand the study and provide consent (or meet the study’s consent rules).
- You take blood pressure medicine at home (you are not eligible if you do not).

This study compares a personalized approach to managing blood pressure during surgery with the usual one-size-fits-all method. It looks at older adults or those with certain heart or blood vessel conditions having non-emergency surgery to see if individual targets can improve recovery.
This study tests whether personalized blood pressure management during and after major surgery can reduce complications and death in older adults with heart or blood vessel risks. It aims to find the safest blood pressure targets for each patient.
This trial studies a way of managing blood pressure during major surgery to help reduce complications. It is for older adults and others at higher risk who will be closely monitored in the hospital.
This trial tests whether keeping blood pressure at 85 mmHg or higher during surgery can reduce confusion (delirium) after surgery. It is for older adults with high blood pressure who are having non-cardiac surgery.
This study looks at whether a simple blood pressure measurement during surgery can predict which older patients may develop high blood pressure after surgery. If you are 65 or older and having planned surgery with a blood pressure monitor placed in an artery, you may be able to join.
This study tests whether an AI tool can help doctors better manage low blood pressure during planned surgeries. If you are having an elective surgery with general anesthesia and need an arterial line for monitoring, this trial may be for you.
