Who can take part
- You have a new diagnosis of rectal cancer proven by biopsy (not a return of old cancer).
- Your tumor is in the lower rectum (less than 16 cm from the anus) and considered “high risk” on pelvic MRI.
- You can start treatment within 6 weeks after your scans, and you are eligible to receive CAPOX chemotherapy (blood counts and organ function must be adequate).
- You are 18 or older, and you can have required MRI and chemotherapy—also including being well enough physically (ECOG 0–1).
- You do not have certain exclusions, like active Crohn’s or ulcerative colitis, known DPD deficiency, pregnancy/breastfeeding, or major heart problems.
