This trial studies whether adding cord-blood–derived cells to ruxolitinib can improve steroid-resistant acute graft-versus-host disease (GVHD) affecting the lower gut or liver. It’s meant for people who are not getting better (or are worsening) despite adequate steroid treatment.

Who can take part
- You’re between ages 12 and 80.
- You have acute GVHD affecting the lower intestines or liver (including cases that appeared after earlier skin-only acute GVHD).
- Your GVHD did not improve after at least 7 days of steroid treatment at the required dose, or it got worse quickly, or it flared while still on steroids.
- A biopsy must suggest GVHD; you do not need a repeat biopsy just to join.
- You must be well enough to participate (performance score at least 30), and you or your legal representative can give consent (and assent if needed).
- You cannot have certain situations that rule you out—like isolated skin-only acute GVHD, ongoing/ongoing treatment with ruxolitinib for more than 96 hours before starting the study, uncontrolled infections, needing more than 6 L of oxygen, or an allergy to bovine/porcine products.
