Who can take part
- Be 18 years or older and able to sign written consent
- Have rectal adenocarcinoma confirmed by pathology (biopsy/test result)
- Have already started FOLFIRINOX chemotherapy
- Have a scan showing the cancer is in the rectum but no clear metastatic (spread) disease on CT
- MRI details must suggest a higher-risk tumor boundary (small CRM) or certain tumor extensions (T3c/T4a/T4b, with exceptions)
- Not have certain heart problems, recent blood clots, pelvic radiation, or conditions that prevent chemotherapy (like DPD deficiency)
