- Be 18 years or older
- You had a kidney transplant within the last year, and your kidney function is fairly preserved (eGFR above 30)
- You have type 2 diabetes and take insulin
- You have home Wi‑Fi to connect the monitoring device
- You must be able to wear the Dexcom G6 sensor all the time
- You must be able to do finger-stick blood sugar testing 4 times per day during the monitoring period
