- Your recent HbA1c (a diabetes blood test) was between 7.0% and 12.5% within the last 90 days
- You must screen positive for diabetes distress (feeling significant emotional burden about diabetes)
- You must be able to do online surveys and a video/telemedicine visit
- You should not plan to move out of the San Diego area in the next 12 months
