Who can take part
- You are a Veteran currently enrolled in VHA care at one of the study sites
- You have one of these conditions: cancer, heart failure, interstitial lung disease, COPD, end-stage kidney disease, end-stage liver disease, or dementia
- Your care risk score (called the Care Assessment Need score) is 90 or higher
- Your clinician is one of the study’s participating VA primary care clinicians who can complete the conversation notes and orders
- You are not a prisoner, not pregnant, and you are 18 years or older
