- You have a confirmed primary kidney disease diagnosis
- You can visit a doctor regularly at a participating clinic (at least every 6 months)
- Your kidney filter function (eGFR) is above 15
- You are at least 2 years old
- You can understand and sign consent to be contacted about research studies
- Your expected lifespan is 6 months or more (not less than 6 months)
