IV clot-busting medicine for stroke when standard options don’t fit

This trial tests whether giving IV TNK (tenecteplase) plus standard stroke care can improve outcomes in some people who have a new stroke but cannot get alteplase (another clot-busting drug) and do not have a large blocked artery that can be treated with a procedure. It’s meant for strokes with “mismatch” findings on scans that suggest salvageable brain tissue.

Who can take part

  • You’re age 18 or older, and the stroke happened recently (you can start treatment 4.5 to 12 hours after the last time you were well).
  • You cannot receive IV alteplase because the stroke started more than 4.5 hours ago.
  • Your scans should show no large/proximal artery blockage that would normally be treated with a procedure (like certain parts of the internal carotid artery, MCA M1/dominant M2, or vertebral/basilar arteries).
  • Before the stroke, you had little or no disability (mRS 0 to 2).
  • Your CT/MRI/angiography and/or blood-flow imaging must show a “mismatch” pattern—small enough core damage and enough affected-but-recoverable brain to meet the trial’s thresholds.
  • A brain scan is needed to confirm there is no bleeding in the brain (no intracranial hemorrhage).
