Who can take part

  • You were hospitalized for COVID-19 and your main diagnosis is COVID-19
  • You are (or will be) discharged straight home, not to a nursing facility or rehab hospital
  • You can walk before discharge, with or without a cane or walker
  • You are 18 years or older
  • You can follow the study plan and use the study’s phone/iPhone coaching
  • You are not pregnant and you don’t have certain unsafe heart or circulation problems
