This study tests whether the drug sitagliptin can delay or prevent type 1 diabetes in people who have a family member with the disease and show early signs of risk. If you are eligible, you would take sitagliptin and have regular health checks for about a year.

Who can take part
- You are between 10 and 45 years old.
- You have a parent, sibling, or child with type 1 diabetes and are between 6 and 45 years old (if first-degree relative) or between 6 and 20 years old (if second-degree relative like an aunt, uncle, cousin, or grandparent).
- You have at least two diabetes-related autoantibodies (a blood test result).
- Your blood sugar levels are higher than normal but not yet diabetes, as shown by a glucose tolerance test.
- If you are female, you must have a negative pregnancy test and agree not to become pregnant for at least one year. If male, agree not to cause pregnancy for one year.
- You agree not to participate in other trials and can delay live vaccines for at least one year after treatment.
