Who can take part
- You have non-small cell lung cancer that is stage III (not suitable for surgery/chemoradiation) or stage IV, or it came back after surgery and can’t be fully cured with other big treatments
- Your tumor has an ALK rearrangement (a specific genetic change)
- After 2–3 months of alectinib, your cancer is not growing (stable disease) or has shrunk (partial response)
- There are not too many active cancer spots—up to 5 lesions on a PET scan—and the team says all spots can be treated with radiation
- You can take the study treatments safely: you’re generally strong enough (ECOG 0–2), have adequate organ function, and—if lung targets are planned—your lung breathing test (FEV1) is at least 1 liter
- You don’t have certain conditions that raise risk: cancer spreading into the brain’s lining (leptomeningeal disease), fluid buildup from cancer that hasn’t settled after induction, or lung scarring/fibrosis or active non-infectious lung inflammation
