- You are age 65 or older
- You were hospitalized for an ischemic stroke (or stroke that came back)
- Before the stroke, you were already taking a blood-thinning medicine (either antiplatelet or anticoagulant)
- You live at home now and plan to go back home
- You agree to participate and allow the interview to be recorded
