Who can take part
- You are 30 years or older (and if you are female, you must be postmenopausal and not pregnant)
- You have diabetes type 1 or type 2 and you are on diabetes treatment (or your blood sugars meet diabetes criteria)
- Your urine test (UACR) is high on at least two mornings, showing ongoing kidney protein leakage
- Your kidney filter rate (eGFR) is above 25, and your A1c is below 10
- You must be able to speak English, Spanish, or Haitian Creole and sign written consent
- If you have type 2 and your UACR is very high (over 300), you must already be on an ACE inhibitor/ARB or an SGLT2 inhibitor
