This trial studies whether injecting botulinum (a medicine that relaxes overactive muscles) into the lower leg can reduce pain and pressure during exercise in people with a confirmed condition called chronic exertional compartment syndrome. It may help by lowering the muscle compartment pressure that builds up after activity.

Who can take part

  • You have a confirmed diagnosis of lower leg anterolateral chronic exertional compartment syndrome (CECS) based on pressure tests after exercise
  • Your post-exercise muscle pressure is very high right after stopping exercise (over 30 mmHg)
  • Your muscle pressure is still high 5 minutes after stopping exercise (over 12 mmHg)
  • If you are female: you are not pregnant, not trying to get pregnant, and not breastfeeding
  • You do not have a history of developing raised, thick scars (keloids)
  • You do not have a significant nerve or muscle disease (for example ALS, motor neuropathy, myasthenia gravis, or related conditions)
