- Be age 40 or older
- Have been discharged from the hospital between 2016 and 2022
- Your discharge diagnosis must be coded as COPD based on your primary diagnosis
- If you have a breathing test (spirometry), at least one test must show a post or pre FEV1/FVC ratio under 0.7
- Your hospital admission should not be for a cause other than COPD
