- A doctor has confirmed you have asthma.
- Your asthma symptoms are not well controlled despite using daily medications like inhaled steroids or combination inhalers.
- You have had recent asthma symptoms (like wheeze or needing rescue inhaler) at least 3 times a week, or have had a flare-up needing oral steroids or hospitalization in the past year.
- You or your child are willing to take part in a registry study that tracks your asthma over time.
