Who can take part

  • You are currently in the hospital or emergency room and plan to go home.
  • You have been diagnosed with heart failure.
  • A recent blood test shows high levels of a heart stress marker (BNP over 200 or NTproBNP over 1000).
  • You are able to take part in the study and don't need to travel far.
  • You do not need dialysis or have very low kidney function (eGFR below 30).
