This trial tests a new combination of drugs (I-DXd and atezolizumab, with or without carboplatin) as your first treatment for extensive stage small cell lung cancer. It has two groups: one for people who haven't started treatment yet, and another for those who've already had 4 cycles of a standard chemo-immunotherapy combination and are stable or better.

Who can take part

  • You must have extensive stage small cell lung cancer (cancer that has spread beyond one lung).
  • You cannot have had prior treatment with certain targeted drugs (like orlotamab or enoblituzumab).
  • You must be willing to have tumor biopsies if you haven't started treatment yet (Cohort 2).
  • You need to be in good physical shape (able to do daily activities, not bedridden).
  • If you have brain metastases, they must be treated and stable, not causing symptoms.
  • You must not have had an autoimmune disease or serious lung inflammation before.
