This trial tests combining two drugs – axatilimab and olaparib (a PARP inhibitor) – for people with advanced HER2-negative breast cancer that carries a BRCA or PALB2 gene mutation. The goal is to see if this new combination is safe and effective.

Who can take part
- You have HER2-negative breast cancer (including low HER2) that has spread or can't be removed, and you have a BRCA1, BRCA2, or PALB2 gene mutation (through blood or tumor genetic testing).
- You have had no more than 3 prior chemotherapy-based treatments for metastatic disease (hormone therapy and targeted non-chemo drugs are not counted).
- You haven't taken a PARP inhibitor before, or your cancer didn't progress while on it or within 12 months of stopping.
- You are 18 or older, able to swallow pills, and have good organ function (like kidneys, liver, and blood counts) – your doctor will check with lab tests.
- You must be willing to have three research biopsies (unless they aren't safe or possible), and if you could become pregnant, you agree to use effective birth control during the study and for 4 months after.
