Who can take part
- You are 30 years or older.
- You had an ischemic stroke (a stroke caused by a clot) confirmed by a brain scan.
- You can do daily activities with some help (Modified Rankin score 3 or less).
- You are enrolling either during your hospital stay or within 24 hours of discharge (early), or between 1 and 6 months after discharge (late).
- You live in one of these states: Virginia, West Virginia, Tennessee, Oklahoma, Illinois, or Michigan.
- You have reliable internet access for video visits and can read and speak English.
