Who can take part
- You are 18 or older
- You were hospitalized for an ischemic stroke within the last 30 days
- You are going home or to an acute rehabilitation facility after the hospital
- You have a smartphone, tablet, or computer with internet
- You can speak English or Spanish
- You do not have severe aphasia (trouble understanding or speaking language)
