Who can take part

  • You have received a letter about your heart disease risk from the SCORE! study before.
  • You have at least one of these: high blood pressure (140/90 or higher), high cholesterol, or high blood sugar.
  • You do not have a history of heart attack, stroke, or blocked arteries.
  • You are not currently in another study that could interfere with this one.
  • You are not pregnant or planning pregnancy in the next year.
  • You do not have a serious illness that would limit your life to less than a year.
