  • You must be 18 or older
  • You spent at least 3 days in the intensive care unit (ICU)
  • You have been diagnosed with moderate-to-severe 'post intensive care syndrome' (symptoms like weakness and trouble with daily activities)
  • You have specific muscle or nerve problems (like polyneuropathy) or trouble swallowing related to your ICU stay
  • You have low exercise tolerance or trouble moving against gravity as part of your recovery
