Who can take part

  • 18 years or older
  • Have type 2 diabetes (or a doctor has told you so, or your blood sugar levels are high)
  • Systolic blood pressure (top number) between 120 and 159
  • Live in a low-income, low-access area of Baltimore (food desert) or a neighborhood with high deprivation
  • Not taking certain diabetes or weight loss medications like GLP-1s, insulin, or similar
  • No serious heart event or hypoglycemia requiring help in the last 6 months
