Tracking What You Eat to Help Your Heart

Who can take part

  • You are at least 18 years old
  • You have been diagnosed with heart failure (NYHA Class II or III, meaning you have some symptoms with activity or at rest)
  • You can use a smartphone or computer app called Cronometer to log the foods you eat
  • You can prepare meals at home and are willing to change what you eat based on a coach's guidance
  • You have not had a heart transplant or a mechanical heart pump (LVAD) implanted
  • Your kidneys are working well enough (eGFR of 30 or higher)
