  • You have type 1 diabetes with large and unpredictable blood sugar swings that have lasted for more than a year.
  • Your blood sugar swings are causing serious problems, like frequent severe low blood sugar events (needing help from others) or dangerous highs (ketoacidosis).
  • You have tried and are using modern diabetes tools (insulin pump, continuous glucose monitor, and a diabetes team) but still have unstable blood sugar.
  • You are not currently using a closed-loop insulin pump system.
  • You have not had a kidney transplant.
