  • You are 70 years or older
  • You have at least one long-term health condition (like diabetes, high blood pressure, or heart disease)
  • You regularly see your general practitioner (GP) for that condition
  • You do not have a dementia diagnosis
  • You do not have known eye diseases or see a private eye doctor now
  • You can understand and speak Danish
