Who can take part

  • You are 18 years old or older.
  • You have advanced heart failure, which means you have severe symptoms (like shortness of breath or tiredness even at rest), you may have a heart pump (LVAD), or you are on a waiting list for a heart transplant.
  • You have had at least one unplanned hospital visit or emergency room visit in the last year that required IV medications.
  • You have a smartphone or tablet that runs iOS 15 or newer (iPhone) or Android 9 or newer.
  • You are able to understand the study and give your permission to take part.
  • You can report your own symptoms and data (like using a phone app) without help from someone else.
