  • 18 years or older
  • Had heart failure for at least 6 months
  • On stable heart failure medicines for at least a month
  • Any type of heart failure (reduced, mid-range, or preserved ejection fraction)
  • Symptoms class I, II, or III (able to do daily activities with some limits)
  • Had at least one hospital stay or urgent visit for heart failure in the past year that needed IV fluids or medicines
  • Able to use a wearable device (patch, watch, or sensor) on your skin
