- Be a patient or willing to become a patient at First Nations Community Healthsource in Albuquerque.
- Have a doctor's diagnosis of high blood pressure (hypertension) that was not caused by pregnancy.
- Speak and read English or Spanish well enough to understand study instructions.
- Have had at least one high reading (top number 130 or higher) in the last 6 months, or get one at a screening.
- Live in Albuquerque, New Mexico, and be able to receive mail there.
