This trial combines three treatments—a targeted radioactive therapy (177Lu-PSMA), a PARP inhibitor (olaparib), and an immunotherapy (pembrolizumab)—to see if they work better together against advanced prostate cancer that has stopped responding to hormone therapy. You may join if your cancer shows strong PSMA on scans and you have already tried certain hormone blockers but your cancer continued to grow.

Who can take part

  • Your prostate cancer has spread to other parts of the body and no longer responds to hormone therapy (castration-resistant).
  • You have taken a hormone-blocking medicine like enzalutamide or abiraterone, and your cancer got worse during or after that treatment.
  • A recent PSMA PET scan shows your cancer has high PSMA levels (a protein on the cancer cells).
  • You have not had chemotherapy for this stage of prostate cancer, nor any PARP inhibitor or radioactive therapy before.
  • You are generally well enough to be up and walking (ECOG 0-1) and your blood counts, liver, and kidney function are within safe limits.
  • You agree to use condoms during sex with a female partner of childbearing potential during treatment and for 3 months after the last dose.
