Early versus rescue plasma exchange for optic neuritis or transverse myelitis

Optic NeuritisMyelitisMyelitis, Transverse

Part of Brain & nervous system, Cancer, Eyes & vision, Immune system & allergy, Infections clinical trials.

This study tests whether starting plasma exchange early (right after steroids) is better than waiting to see if steroids are enough for people with severe optic neuritis or transverse myelitis. It may help preserve vision or movement by stopping inflammation sooner.

Who can take part

  • At least 18 years old
  • For optic neuritis: severe vision loss (20/200 or worse) and an MRI showing optic nerve inflammation, within 14 days of symptoms
  • For transverse myelitis: spinal cord inflammation with significant disability (EDSS score of 3 or more) and onset of motor symptoms within 14 days
  • No prior optic neuritis in the affected eye (for optic neuritis sub-trial)
  • Not pregnant and no medical conditions that would make plasma exchange unsafe
  • No recent use of immune-suppressing drugs or certain other treatments (like steroids, plasma exchange, or investigational agents)

Clemastine to help restore nerve covering in optic neuritis

This Phase 2 trial tests whether clemastine can help regrow the protective covering around the optic nerve after acute optic neuritis (an inflammation-related vision problem). It may help improve vision recovery and signal how well “remyelination” happens.

Testing a new drug for sudden vision loss from nerve inflammation

This study tests whether a new medication called privosegtor can help restore vision in people who recently experienced sudden vision loss in one eye due to optic nerve inflammation. You would receive the study drug early after your symptoms start, when treatment may be most effective.

Visual pathway study for recent optic neuritis

This study looks at how the eye-to-brain visual pathways work in people who have had optic neuritis symptoms for less than 4 weeks. It may help researchers better understand neuro-inflammatory (nerve inflammation) disorders and improve future care.

Efgartigimod for first-time optic neuritis

This trial tests a drug called efgartigimod for people having their first episode of optic neuritis (inflammation of the optic nerve). The goal is to see if it can help protect your vision better than standard treatment alone.

Testing the 2022 optic neuritis diagnostic criteria

This study is checking whether a new 2022 set of diagnostic rules correctly identifies optic neuritis, which is inflammation of the nerve that connects your eye to your brain. If you've recently had vision problems and doctors suspect this condition, participating could help confirm the diagnosis and shape future care.

NCT07595965Not yet recruiting· Phase 1
Stem cell treatment for severe neuromyelitis optica relapse

This trial tests a new stem cell treatment for people with a severe relapse of neuromyelitis optica (NMO), a rare autoimmune disease that affects the eyes and spinal cord. The treatment may help reduce damage and improve recovery.
