  • You are between 45 and 70 years old.
  • You live in one of these Pennsylvania counties: Philadelphia, Bucks, Delaware, Chester, or Montgomery.
  • You have prediabetes or diabetes and are a Penn Medicine patient.
  • You eat fewer than 5 servings of whole grains each day.
  • You use online grocery shopping at least once a month and have a credit or debit card to pay.
  • You have reliable internet access and speak English.
