- Your baby was born early (before 32 weeks) or weighed less than 1500 grams at birth.
- Your baby had a brain bleed (like periventricular hemorrhage or intracranial hemorrhage grades 2, 3, or 4).
- Your baby had low oxygen at birth or other serious newborn problems (like HIE stage 3, kernicterus, perinatal stroke, or asphyxia).
