Who can take part

  • Your baby was born early (before 32 weeks) or weighed less than 1500 grams at birth.
  • Your baby had a brain bleed (like periventricular hemorrhage or intracranial hemorrhage grades 2, 3, or 4).
  • Your baby had low oxygen at birth or other serious newborn problems (like HIE stage 3, kernicterus, perinatal stroke, or asphyxia).
  • Your baby had severe infections (like sepsis from gram-negative bacteria, necrotizing enterocolitis, or infantile apnea).
  • Your baby had a low Apgar score (3 or below) at 5 minutes, or was diagnosed with growth problems in the womb.
  • Your baby needed breathing help (oxygen or a machine) for a long time, or had retinopathy of prematurity.
