Who can take part

  • You have been diagnosed with chronic heart failure
  • Your heart function (LVEF) is 49% or less, or you have been told you have heart failure with preserved ejection fraction (HFpEF)
  • You are 18 years or older
  • You own a pedometer, smartwatch, activity tracker, phone app, or step counter
  • You have not been in the hospital for heart failure in the last 3 months
  • You do not have a disease or disability that stops you from being physically active
