Who can take part

  • You are 18 years or older.
  • Your heart's pumping strength (ejection fraction) is 50% or higher.
  • You have been diagnosed with HFpEF or have at least two risk factors like diabetes, high blood pressure, obesity, or being inactive.
  • A test shows your physical function or exercise ability is below normal.
  • You can use a cell phone and a mobile app.
  • You have not been hospitalized in the last month and do not have frequent falls or unstable weight.
