- You are between 40 and 85 years old.
- You are being discharged home from a hospital in Alberta after being treated for diabetes, coronary artery disease, heart failure, chronic kidney disease, or hypertension.
- No one else in your home is already in the study.
- You are not being discharged to a nursing home, rehab facility, or another hospital.
- Your care goals at discharge are not focused only on comfort (palliative care).
- You have the ability to make your own medical decisions (no legal guardian or decision-maker assigned).
